As a Pharmacist, you will play a critical role in providing pharmaceutical care to patients and ensuring the safe and effective use of medications. Your responsibilities will include dispensing prescription medications, providing drug information and counseling to patients, and collaborating with healthcare professionals to optimize patient outcomes. The ideal candidate will have strong clinical knowledge, excellent communication skills, and a commitment to delivering high-quality pharmacy services. Key Responsibilities: Medication Dispensing: Dispense prescription medications accurately and efficiently, following pharmacy protocols, dosage instructions, and patient-specific requirements. Verify prescription details, dosage instructions, and potential drug interactions to ensure the safe and appropriate use of medications. Patient Counseling: Provide medication counseling and patient education on proper medication use, dosage administration, potential side effects, and drug interactions. Address patient questions, concerns, and inquiries regarding their medications, health conditions, and treatment plans. Medication Management: Monitor medication adherence and compliance among patients, identifying and addressing potential barriers to medication adherence, such as cost, accessibility, or adverse effects. Collaborate with healthcare providers to review medication therapy regimens, recommend therapeutic alternatives, and optimize drug therapy outcomes. Medication Safety and Quality Assurance: Ensure compliance with pharmacy regulations, policies, and quality standards to maintain medication safety and prevent medication errors. Participate in medication reconciliation processes, medication reviews, and adverse drug event reporting to promote patient safety and quality of care. Inventory Management: Manage pharmacy inventory, including medication stock levels, expiration dates, and storage conditions, to ensure adequate supply and minimize waste. Place orders for medication replenishment, verify incoming stock deliveries, and maintain accurate records of inventory transactions. Health Promotion and Public Health Services: Provide health promotion and disease prevention services, such as immunizations, smoking cessation counseling, and medication therapy management programs. Collaborate with community health organizations and healthcare providers to support public health initiatives and promote medication safety awareness. Qualifications and Skills: Bachelor of Pharmacy (B.Pharm) or Doctor of Pharmacy (Pharm.D) degree from a recognized institution. Registered Pharmacist with the Kerala State Pharmacy Council or eligible for registration. Strong clinical knowledge of pharmacology, therapeutics, and medication management principles. Excellent communication, counseling, and interpersonal skills. Proficiency in pharmacy software systems for medication dispensing and documentation.
