Monitor patients' progress regularly and modify treatment plans as necessary to achieve optimal outcomes. Maintain accurate and up-to-date documentation of patient evaluations, treatment plans, progress notes, and discharge summaries. Collaborate with other healthcare professionals, such as physicians, occupational therapists, and nurses, to coordinate care and ensure holistic treatment approaches. Participate in interdisciplinary team meetings, case conferences, and continuing education activities to enhance clinical skills and knowledge.
